Just click the "New" button on the main toolbar and then select the "DVD with DVD case" option from the "Create a New Label" dialog that pops up. If you've got version 1.38 or higher of Acoustica CD/DVD Label Maker, you can also select the "Instant-Label Wizard" option from the "File" menu and then select the "DVD with DVD case" option from the "Choose a label type" page of the wizard.
Once you've done this, you should see tabs labeled "DVD", "Case", "Insert Front", and "Insert Back" at the top of the label window. Click on the "DVD" tab to design a DVD disc label, or click on the "Case" tab to design a DVD case insert of the type normally used with movies. This label will print at a size of approximately 274 x 184 millimeters (10.8 x 7.250 inches), with possible slight variations in size depending on the kind of paper stock you choose to print on. Click the "Insert Front" or "Insert Back" tabs to design the sheet that clips inside the DVD case.
